Step 3: Have Your Badge Made and Complete Your I-9 Form

Before you begin work you need to have your identification badge made. Your recruiter will tell you when and where to have your badge made.

Carilion uses the E-Verify work authorization system created and maintained by the U.S. federal government. To verify your employment eligibility, you need to complete an I-9 form. To complete the form:

• Print out the form in Onboarding Express.
• When you come to have your identification badge made, you will need to bring documents with you to complete the I-9 employment eligibility process. View a list of acceptable documents (.pdf). If you choose to provide a document from list B, it must contain a photo of you. 
• Please note: Employees may present one selection from List A OR a combination of one selection from List B and one selection from List C from the List of Acceptable Documents for I-9 Verification.
