Step 2: Visit Employee Health

Before you begin work, you must complete a post-offer assessment (POA). Requiring a POA for everyone helps us ensure that employees are able to perform their jobs and provide the care and services our patients need. The POA is confidential—Employee Health will not share specific information with Human Resources or your manager or director.

What's Involved in a POA?

  • A drug screening to comply with Carilion's drug-free workplace policy.
  • A PPD skin test (also known as TB skin test), blood test and/or chest x-ray. Employee Health staff will let you know if a PPD is required for your position.
  • Fit testing for a TB Respirator mask for those who might care for a positive or suspected TB patient or patient who is suspected of having flu or other airborne communicable diseases.
  • A thorough discussion of the physical requirements and job functions of the job you have been offered and a determination of your ability to perform those functions.
  • A thorough analysis of your immunization status and need for booster shots or titers to be completed. A condition of employment with Carilion is your up-to-date immunization against the measles, mumps and rubella viruses.

When is My Appointment, and Where Do I Go?

  • Your recruiter will schedule your Employee Health visit.
  • For directions, view our list of Employee Health locations.
  • Plan to arrive 15 minutes before your appointment; this allows for delays in parking, traffic, etc.
  • If you are running late or need to reschedule your appointment, please call Employee Health, and we will make every effort to assist you. 
  • If you are more than 20 minutes late for your appointment, you may need to reschedule.

What Do I Bring?

When you visit Employee Health, please bring the following with you to your appointment:

  • Your Step 1: Employee Health forms that you printed from Onboarding Express.
  • A valid, unexpired driver's license or other federal or state-issued picture identification.
  • Your immunization records including documentation of prior PPD skin tests, MMR (measles, mumps, rubella vaccine), tetanus status, hepatitis B vaccine history and varicella (chicken pox) history.
  • Titers indicating immunity are acceptable. Suggestions for places to obtain vaccine records include: previous healthcare employer, grade school or highschool, military records, primary care physician's office.
  • All prescription medications you have taken in the last 30 days. Please bring the original prescription bottles so that we may verify the medications.
  • Eyeglasses or contact lenses if you wear them.
  • A list of all over-the-counter medications you have taken in the past three weeks.
  • Parental Consent Form (.pdf) if you are under the age of 18.

Important Note About Your Employee Health Visit

For safety and compliance reasons, our Employee Health offices are designed to provide services and care to our employees, including new hires, only. For their safety and comfort, please do not bring children of any age to your Employee Health appointment. We are unable to provide childcare during your visit and will reschedule your appointment to a more convenient time if you are accompanied by children.
